In your purchase confirmation email you will receive a letter of agreement. Please sign it and email it to Marissa at mlustri@opha.on.ca. Once the agreement has been received, we will contact you to provide your organization’s logo and/or contact information that you would like to be added to the resource. You will then be emailed a copy of the high resolution PDF file with the discussed customizations. With this license you have the ability to share the electronic resource. If you would like to print this resource and distribute it, we ask that you please purchase the license to do so for $100 (one time fee).
